Background and Abuse Prevention Training Screens

Use this form to start the background screen and APS training process. Background screen/APS training is $38 and are good for two years. JVA is committed to creating a safe and positive environment for its participant’s physical, emotional, and social development and to ensure that it promotes an environment free from abuse and misconduct. As part of this commitment JVA will be following the recommendations and guidelines of the Abuse Prevention Systems Please read through each page carefully to ensure that all information is provided accurately. DO NOT COMPLETE THIS FORM FOR ANYONE OTHER THAN YOURSELF! If you have a current AAU background screen, you do not need to complete this frorm.

ONLY COMPLETE THIS FORM ONCE!! Completing it multiple times triggers multiple screens to be sent to you. After you submit this form, two emails will be sent to the email that you provided. You will be able to complete your background screen and APS training via the links provided in the emails. If you are a JVA member club, the status of your background screen will show on your club page.
