Background Screen and Trusted Coach Training Form

Use this form to start the background screen and training process. Background screen and training is $27 and is good for one year. DO NOT COMPLETE THIS FORM FOR ANYONE OTHER THAN YOURSELF! If you have a current AAU background screen, you do not need to complete this form.
